care What You’ll Do: Provide individualized speech, language, and
swallowing therapy in clients’ homes Conduct diagnostic assessments
and create tailored treatment plans Offer functional communication
and cognitive-linguistic training Educate clients and caregivers on
strategies that reinforce progress and safety Document services
using EMR and communicate with the care team as needed
Qualifications: Current Minnesota Speech Language Pathologist (SLP)
license Valid driver’s license and reliable transportation
Experience with EMR preferred Passion for patient-centered,
